2. Provide your full name, email address, phone number, address, and date of birth. *If you are selected as a winner, you are required to provide proof of age to collect your prize.

3. Submit your entry by clicking the “Submit” button.

Each valid submission via the online form counts as one entry into the giveaway. Participants can submit one entry per week through this method.

Limited to one entry per person per week via AMOE. Multiple entries will be disqualified.
